Patients who experienced removal of their RC battery were less likely to get a new battery to replace it than patients with PC batteries. The replacement rate at seven years was higher for PC (33.7%) than RC (29.5%) by 4.2 percentage points (log-rank χ 2 = 219.2, df = 1, p < 0.001).

Explant rate at seven years was lower for PC (19.2%) than for RC (22.0%) systems by 2.8 percentage points (log-rank χ 2 = 25.0, df = 1, p < 0.001 Fig. 4).

The composite endpoint of survival free from IPG removal for any reason was divided into two competing endpoints, IPG explants and replacements. The proportion of implants in the study that were PC was 22% in 2013 and grew to 29% by 2017 ( Fig. 2). The distribution of primary pain type was clinically similar between groups, with the most common indications being failed back surgery syndrome, neuritis/radiculitis, and back pain. Notably, patients implanted with PC devices were older, less likely to be on Medicare owing to disability, and less often de novo. All variables were statistically different between PC and RC groups. The characteristics of the patient at the time of implant are shown in Table 3. The average follow-up time was 2.7 ± 2.0 years for RC and 2.4 ± 1.8 years for PC IPGs. Of the 119,628 implants performed in the outpatient hospital, 79,606 were RC, 26,856 were PC, and 13,015 could not be identified and were excluded. Implants occurring in the inpatient hospital (5%) and ASC (37%) were excluded because of the inability to classify the battery as RC or PC.

Therefore, the cooccurrence of an IPG explant code and a lead explant code on the same day was required to define an explant in the outpatient hospital and ASC.Ī total of 207,188 chronic pain neurostimulator implant procedures were identified in the Medicare FFS claims between January 2013 and September 2020 ( Fig. 1). The HCPCS code 63688 can be used to describe either an IPG explant or an IPG revision. After the identification of replacements, the remaining claims were searched to identify explants by HCPCS code or ICD-9-PCS/ICD-10-PCS code for IPG explant.

In the inpatient hospital setting after October 2015, a replacement was defined by the cooccurrence of an ICD-10-PCS IPG explant code and an ICD-10-PCS IPG implant code on the same claim. Replacements were identified by HCPCS code or International Classification of Diseases, Ninth Revision Procedure Coding System (ICD-9-PCS)/International Classification of Diseases, Tenth Revision Procedure Coding System (ICD-10-PCS) code for IPG replacement ( Table 1). Claims from any site of service, including inpatient hospital, outpatient hospital, ASC, and office, were included. Outcomes of interest were identified from Medicare claims that occurred after the index date based on procedure codes ( Table 1).
